The Science Behind Myo-Inositol

If you’ve Googled “natural supplementation for PCOS” recently, chances are you’ve come across Myo-Inositol. If you’re unsure what this compound is and what it actually does, stick around. 

Myo-inositol belongs to a family of naturally occurring sugar alcohols called Inositols, which function similarly to B vitamins in the body. These compounds play a key role in cellular signaling, particularly in processes related to insulin action, hormone regulation, and ovarian function.

There are nine inositol isomers, but the two most studied in the context of PCOS are Myo-Inositol (MI) and D-Chiro-Inositol (DCI). While both are involved in insulin signaling, they serve different biological functions. MI is primarily responsible for cellular glucose uptake and FSH (follicle-stimulating hormone) signaling, both of which are vital for healthy ovulation. DCI, on the other hand, is more involved in glycogen synthesis and androgen regulation.

Under normal conditions, the body can convert MI into DCI in specific tissues. But in women with PCOS, this conversion can become disrupted, leading to an imbalance that may contribute to insulin resistance and abnormal ovarian function. Researchers are especially interested in MI because insulin resistance is a core feature of PCOS—even in women who aren’t overweight. By improving insulin sensitivity, it could be possible to address a wide range of PCOS symptoms, from irregular cycles to anovulation (no egg released during ovulation) to acne and hirsutism.

One major study from 2017 reviewed several clinical trials and found that Myo-inositol (MI) helped lower insulin levels, improved overall metabolism, and reduced levels of male hormones like testosterone. This is important because too much insulin in the body can trigger the production of excess androgens (male hormones), which can disrupt ovulation and make PCOS symptoms worse. A more recent review in 2023 backed this up. It showed that MI reduced both total and “free” testosterone, and increased a protein called SHBG, which helps keep male hormone levels in check. In simple terms, this means improved hormonal balance

One of the most encouraging discoveries about Myo-Inositol (MI) is its ability to support ovulation and improve fertility. Several large reviews of clinical studies have shown that MI helped women with PCOS ovulate more regularly, improve egg quality, and potentially boost pregnancy rates—whether you're trying to conceive naturally or going through IVF.

While the data on MI is promising, it isn’t without its caveats. Not all outcomes are uniformly improved. For instance, while ovulation rates and embryo quality often improve, clinical pregnancy rates in IVF settings don’t always show consistent gains.

It’s important to note that many of these studies were completed on a small number of people and not all study subjects or methods were standardised therefore the studies differ widely in dosage, treatment duration, patient characteristics, and whether MI was used alone or in combination with D-chiro-inositol. This variability means that we can’t assume that the outcomes in these studies can be applied to everyone. 

Finally, a 2024 review emphasised that although evidence is trending positive for the use of Myo-Inositol, it remains “limited and somewhat inconclusive” when applied to broader PCOS outcomes like long-term weight management or cardiovascular risk.

Inositol—particularly Myo-Inositol—has emerged as one of the more promising natural options in the PCOS space. While the research is still developing and not all outcomes are consistent, the evidence to date suggests Myo-Inositol may support hormonal balance, insulin sensitivity, and ovulatory function in women with PCOS.

Perhaps one of its greatest advantages is its strong safety profile. With minimal reported side effects and a growing base of clinical support, it's no surprise that many women turn to Myo-Inositol as part of their PCOS management approach.
